Carers Allowance Confirmation Form
The Warmer Homes Scheme is a nationwide scheme delivering free energy efficiency improvements
to the homes of people who are in receipt of certain welfare payments. The scheme is open to
individuals in receipt of the carers allowance who live with the person they are caring for.
This form confirms that the individual in receipt of carers allowance is residing at the same property
as an individual they are caring for.
It should be completed by an employee of the Department of Employment Affairs & Social Protection
(DEASP) and submitted to SEAI as part of your application for the Warmer Homes Scheme. The form
should also be signed, dated and stamped by the employee of DEASP.
