CREATE A FORM IN GOOGLE FORMS

1. Access Googles Form using your Gmail

account.

3. Provide a title and description. (a)

4. Select type of questions (e.g., Multiple

5. Add more questions (if necessary) (c)

6. Add title and description (if necessary) (d)

7. Add image (if necessary) (e)

8. Add video (if necessary) (f)

9. Add section (if necessary) (g)

* Note:

- You can duplicate the question. (h)

- You can delete the question. (i)

- You can make question required. (j)
